COVID-19 FORMS

Both new and existing clients will need to complete the Informed Consent and Waiver form prior to coming in for your appointment. You will only need to sign this once as it covers all future visits throughout the pandemic period. All clients will need to complete and return the Daily Questionnaire BEFORE coming in for each appointment.
